Carl M. Formhals (c.1858-1908) was born in Ottawa, Illinois, and died in New Mexico. He was a machinist who became interested in Christian Science in early 1886 while living in Aurora, Illinois. In March 1886 he wrote to Mary Baker Eddy, ordering two copies of Science and Health with Key to the Scriptures.
